Medication Authorization Form

This form MUST be filled out and signed every time you bring in medication for your child. 

Steps to take when bringing in medication:

1.  Place your child’s medication in a zip lock bag
2.  Medication “MUST” be in original container
3.  Label with the child’s first and last name along with classroom
4. Fold form and place in zip lock bag
5. Give medication to the office staff (medication can “NOT” be in the classrooms)
6. Let “ALL” teachers in the classroom know there is medication in the office to be given to your child
